1. Problem Statement: Increasing number of flight accidents.
Safety is an important issue for the aviation industry. The functions of the aircraft depend on the automated machines. However, safe flying of an aircraft also depends on the pilot (De Felice,Petrillo & Zomparelli,2016). Miscommunication between the pilot and air traffic control can cause the accident.
2. Problem challenges:
Most airline companies do recruit pilot with high experience (Bevilacqua & Ciarapica, 2018).Human error cannot eliminate, so there is a chance that an experienced pilot can make a mistake while handling a challenging situation (Joe, 2014).Miscommunication can happen in two ways- skill-based miscommunication and fault in auditory sensory processing.
3. Problem aspect (human-artefact): Miscommunication between the pilot and air traffic control.
The reasons for the problem are lack of attention and fault in auditory sensory processing (Barzilla & Lee,2017). Miscommunication between a pilot and air traffic control is related to wrong interpretation of the instructions, which can happen from both ends.
4. Miscommunication between a pilot and air traffic (fault in auditory sensory processing):
In 1989, Boeing 747 of Flying Tigers crashed into the runway in Kuala Lumpur killing four people (Holloway & Eloranta, 2014). The crash was due to the fault in auditory sensory processing. Air traffic control gave clearance “descend two four zero zero” (2400 Ft.), while pilot interpreted it as “descend to four zero zero” (400 Ft.).

Boeing 727-46 jet aircraft of Dan-Air was flying to Tenerife from Manchester in 1980.The flight crashed because of the miscommunication between the pilot and air traffic control.(Abdullah, Takahashi& Lakhani, 2017, March).During the investigation, it was found out that pilot interpreted the word “inbound” as the “outbound”.
